Always professional, top notch quality of care

Your green plastic "check in " information computer notebook is not an efficient format for your patients to give their medical/health history.Too much non-relavant information wanted unrelated to the problem. Not enough flexibility in answering questions.Really an inadequent approach for the results that you want.It's terrible for the patient to try to get through the process; it's so difficult & inadequate for patient responses.PLEASE, create a better way for all concerned.Do not ask for the patients "real" Social Security Number. There is a 2nd one SS has given US citizens. Use that.I've had several doctors offices patients SSN and medical records stolen, which included mine also!
