I checked no for question # 6 because nobody in management will contact me. For almost 4 years of my father’s stay, when I left a message for management, the call was never returned. Maybe once there was a phone call back with a message which I promptly made a return call. They never called back.

The communication regarding my father's discharge were frequent and very clear. To add an additional layer of complication, he had an offsite doctor's appointment the morning he was discharge. It all went seamlessly with transportation to the doctor, where I met him, the back to his room in building 3 with all of his things. The only improvement that I could see was coordination with the doctor in rehab. She had ordered a test the day before discharge and called me with the results the evening after he was discharged, but there was nothing I could do with the results as I am not a medical professional. I realize that these things happen, but it could have been better coordinated.

Upon discharge, there was no discussion or explanation of some changes made to the medications. We were just handed a bag of left over medications. If I hadn't known about the one new drug and what needed to be done with it, I would have just given it at potential risk of adverse effects. Also, no discussion about his wounds and what needed to be done next with them. Also clock and TV did not work the entire time he was there.

I am comparing Country Meadows with Complete Care of Brakley Park and home care. Home care was more personal and professional.

I felt the communication from admin was sub-par. I wasn't notified of changes in a timely manner. Ended up staying extra days due to transport issues.

Loved one died so it wasn't technically a discharge. We gathered everything the night she passed away and moved everything out.

My mom has only been there for 3 days and so far it has been awful. When transport brought her in they left her completely unattended for an hour. When the "nurse" finally came in she said oh. I didn't know you where here. And believe it or not it has gone down hill from there. Our experience has even included theft. Just an awlful awlful place!
