Thank goodness I arrived when I did on my dad‘s admission day. He was sitting in his wheelchair, bent over, trying to unpack his bag… He could have toppled right out of his wheelchair. It has been very difficult to get in touch with anyone there, and after 3.5 days there,we do not know the plan for his stay at your SNF.

I was told on Monday my father would be discharged either Tuesday or Wednesday, but “I’ll call you either way to let you know”. I waited Tuesday for a call. I couldn’t spend the whole day there, but wanted to be there to help him settle in back in bldg 3. I assumed he wasn’t d/c’d because I didn’t get a call. I went to the rehab unit at 10 am Wednesday and found out he was discharged the day before. All of the bags with his belongings were heaped by his bed, untouched for 24 hrs ! I’m thankful he didn’t fall getting his feet caught on them getting in/out of bed until I got there to put his toiletries and clothes away, which had powder all over them from an open container.

Zero communication with PT apon discharge. Grandmother almost fell returning home. Sent back to hospital with low temperature and low blood sugar. Zero structure and guidance for patients. Waste of time and money.

Give a week and half notice as opposed to a week for date of discharge due to weekends so for example the Thursday and Friday before the next week tell families discharge date so people can prepare better.

We were disappointed in staffing level. Communication was an ongoing problem. There were many times that information was not properly passed on. Many times information was forgotten. For example my mother was there for rehabilation with a broken left arm. We were told to bring clothing that buttoned down the front. We were told that a button down shirt would not work. Instead she was dressed in a pullover sweatshirt.

Check to make sure they fully understand the discharge papers, ask them questions.
