My mom came to the skilled nursing from assisted living both at Bethlehem Country Meadows and it didn’t feel like the new staff were aware of this. Some of the staff are not very friendly to my mom. We have yet to have any contact from any of the therapists

As my father was a resident at the Country Meadows Allentown facility, prior to his hospital stay, there seemed to be little communication between the facilities as to his previous care at Allentown and care at the hospital.

The walker order should have been placed sooner so we had it on the day of discharge.

Overall, my mom‘s stay was better than we’ve experience in the past at other subacute rehab centers. That being said much was left to be desired on her actual discharge day. The planning and the communication prior to the discharge day was great and the family meeting was much appreciated and informative. On the day of discharge, the discharge plan changed and it wasn’t until after she left the building that the doctor called me to tell me that everything changed. My brother was already in the car with my mom on the way back home. The doctor refused to provide medical clearance for outpatient physical therapy at the last minute. I understand the reason reasons provided, but that should’ve been provided well before discharge.
