Effective communication is crucial. When the family leaves multiple messages, even if there is no update to provide, return their call and inform them of this. This approach is preferable to making the families feel disregarded.

Discharge went smooth. Maybe more communication leading to discharge dates so that families are more prepared for next steps to care for patient.

Communication with staff and Mom's nurses is our #1 priority. This has not been super smooth especially when trying to have mom advocate for herself. I'm sure as they get to know her, and us as a family, this will improve.

MY MOM PATRICIA STROELL WAS VERY COMFORTABLE WITH THE SERVICES SHE RECEIVED AT COUNTRY MEADOWS. SHE WAS SAD TO GO BUT KNEW IT WAS TIME TO GO HOME.
