Upon discharge, there was no discussion or explanation of some changes made to the medications. We were just handed a bag of left over medications. If I hadn't known about the one new drug and what needed to be done with it, I would have just given it at potential risk of adverse effects. Also, no discussion about his wounds and what needed to be done next with them. Also clock and TV did not work the entire time he was there.

Loved one died so it wasn't technically a discharge. We gathered everything the night she passed away and moved everything out.

My mom has only been there for 3 days and so far it has been awful. When transport brought her in they left her completely unattended for an hour. When the "nurse" finally came in she said oh. I didn't know you where here. And believe it or not it has gone down hill from there. Our experience has even included theft. Just an awlful awlful place!

My mother was admitted to your rehabilitation facility on Friday.Your facility put her in a room with a screamer. It went on a night longWhy in the world would you do that.I understand the lady has dementia, but how dare you put my mom in that room. What you showed me was you didn’t care about my momIt was another person to put in a bed so you can collect moneyWhen I got there on Saturday, I only had to sit for 10 minutes and this lady went on and on and on at the top of our longThe nursing staff was nice and got my mom moved within 10 minutesSeriously, I would’ve never picked this facilityAnd I will make sure people know about itMy mother’s health and her well-being matters to us tooShe’s not a number and more money for you

Thank goodness I arrived when I did on my dad‘s admission day. He was sitting in his wheelchair, bent over, trying to unpack his bag… He could have toppled right out of his wheelchair. It has been very difficult to get in touch with anyone there, and after 3.5 days there,we do not know the plan for his stay at your SNF.

I was told on Monday my father would be discharged either Tuesday or Wednesday, but “I’ll call you either way to let you know”. I waited Tuesday for a call. I couldn’t spend the whole day there, but wanted to be there to help him settle in back in bldg 3. I assumed he wasn’t d/c’d because I didn’t get a call. I went to the rehab unit at 10 am Wednesday and found out he was discharged the day before. All of the bags with his belongings were heaped by his bed, untouched for 24 hrs ! I’m thankful he didn’t fall getting his feet caught on them getting in/out of bed until I got there to put his toiletries and clothes away, which had powder all over them from an open container.

Zero communication with PT apon discharge. Grandmother almost fell returning home. Sent back to hospital with low temperature and low blood sugar. Zero structure and guidance for patients. Waste of time and money.
