Overall the experience has been positive. The staff is very attentive and respond quickly to patient requests. The food has been excellent — far better than other assisted living/Rehab experiences I’ve had. The patient is comfortable and feels well cared for.

Clothing and personal belongings were already organized and ready to go when I arrived. Staff was available for questions at that time and assisted with moving belongings to our car. Staff was very supportive with kind words regarding my mother as we left. Only thing I can add for improvement, was that I wish I had been contacted after my mother's passing with an expectation for her room cleanout such as timeline, what items could remain behind for trash, etc. I had to call.

I am describing my first night at Country Meadows after being released from St. Lukes Anderson as unsatisfactory since upon arrival I was placed in a room with a patient who shouted nearly the entire night. Two days later I was moved to another room with a roommate who wailed loudly, scratched and bit the attendants and once again I barely had a chance to sleep until her family moved her to another facility. During my stay there I had some excellent nurses, attendants, and therapists. The food was also mostly excellent.The following people were among the finest in delivering care: Lou (evening attendant), JoAnn (morning nurse), Haley, Diane, & Marcello from P.T., Otis (very kind, thoughtful van driver) and many other therapists and attendants who I may not have gotten their names.My suggestion would be for patients entering your facility for therapy should be given a room where they can get a decent nights sleep.

Many people helped with transitioning mom from rehab into independent living. The one person that really stood out was Lien Snyder.

My only real negative comment is the fact that when my uncle needs rehab, he is sent to the Bethlehem facility and still gets charged for residing at the Allentown facility. I don't understand why he is charged for both facilities. I would appreciate some insight on that.

We would like updates on care at least weekly. Three weeks is too long. Also we don’t know who to speak to inorder to get updates and ensure that our family member needs and conditions are known. I put a whiteboard in her room so that everyone could see it as opposed to clinical notes that are somewhat hidden and it’s up to the individual provider to log in to a computer and read the notes. So far care seems to be good. Unfortunately there are no stimulating activities in the afternoon and evening. “Meditation and Relaxation” are not what my mom needs. She loves word games and any other activities that stimulate the mind. I received a call from social work and after making a call got feedback from therapy as well. I ran into cognitive therapy on a visit. All those interactions were good. The Care Agreement took about 5 phone calls and days for me to receive it. There was an error on it and that took a few emails to get corrected.
