First I would like to explain that when my mother came to live at Country Meadows she was in the independent living section of Country Meadows. She was very pleased with her surroundings there and had many friends, some old friends, and some new ones. She often referred to her residence there as “the country club” and because she was such a sociable individual she was very much at home in that environment. However as circumstances dictated and her health began to decline, she moved to the assisted living facility of Country Meadows which was conveniently located next door. The transition was seamless and our experience with this facility of Country Meadows, and the care they provided for my mother, was outstanding. Every aspect of her care was superlative. The aides were caring and truly loved my mother. This was very important to me as my sister and I both live out of state and had long drives which made it difficult to visit her more than twice a month. So it was very comforting to us that we felt she was in an environment that was a second home for her. One of the reasons I felt so strongly about the level of her care is because whenever there was a change in her circumstances, I knew I would be contacted by someone at Country Meadows to inform me regarding her particular situation. And this was the case up to the very end of her life. Anyone who asks for a recommendation for a facility for an aging parent will most certainly receive my recommendation for Country Meadows.

I loved my dad being there. It was so sad that his stay there was so short. I felt good leaving him in your care. He was not there long enough to participate in everything you provided.

From our initial inquiries about moving into CM until my mother's final days at CM, all of our experiences have been excellent. The financial structure of monthly payments and 30 days' cancellation provided comfort from a financial aspect and there was never any pressure to make a decision. Marnie was extremely helpful and extremely patient with the move-in decision. In her last few years, my mother lived in Connections/Memory Care. The staff was amazing in their compassion and patience. They were communicative about her health and provided excellent care. The apartment and the entire complex were always extremely clean and attractive and all staff members friendly, professional, knowledgable and helpful. My sister had experiene with expsive memory care units in NY state wtih her mother-in-law and states that those facilities could not compare in any favorable way to CM.

I did extensive research prior to moving my mom to Connections at Country Meadows. First and foremost, I could tell there was a sense of community and the staff truly cared about the residents. CM had the nicest rooms and may seem trivial, but I liked the shadow boxes outside the individual’s rooms. That not only served as a marker for the resident, but also showed staff and others the type of interests the person had prior to cognitive decline. Having access to a secure area outside was another selling factor. This was the absolute all around best place and I was sad to have to leave. Seriously, staff is the BEST!

Maybe provide a checklist of tasks required for a complete and smooth transition - things like cancel utilities and phone, change address with post office, newspaper, and bank etc, set up appt with cable company if additional package desired, etc.
