This survey answers apply to the Forks Country Meadows only. Everything at Forks was above excellent accommodations. What we experienced at the Bethlehem Country Meadows was awful. There are so many bad things that went on there the 1-3 question would be poor and I would never recommend the Bethlehem facility to anyone. The PT at Bethlehem was fine. The move to Pathways was ok at first but it really went horrible after the 2nd week. There were a few that transferred to other faculties in the short time (2 Months) that my husband was there. Most of them for the same reason I moved him. There were some very good aides at Bethlehem but also some bad ones especially on 11-7 Going to the Forks was a blessing. I could now go home andknow he was given excellent care and sleep at night. The next day I could come to visit and not have a complaint. Almost every day when I came to visit at Bethlehem, I had to complain about the same problem again. He had no catheter and he would ring the bell and no one would come leaving him with wet clothes. I would be all day listing the complaints. Something needs to be done there. For what we pay I felt he deserved better care. Again, the care at Forks was worth every penny paid. I wish Bethlehem would have been the same. Again, there were some excellent aides but they were not always assigned to him or on duty. If you want to rate this survey make sure the Forks gets the excellent rating and Bethlehem the poor rating

Country Meadows is all that was promised at the initial meetings and intake conference.We experienced a thorough, efficient, professional team that we desired for our loved one. Kind, compassionate, and offering a level of dignity that we requested and appreciated that was second to none.Thank-you for seeing us through this very difficult end of live journey.

Organize a meet and greet for residents and their families to introduce direct contact staff, such as the medical team, activities coordinator, maintenance. This would help the new residents acclimate better in a new environment.

So far I cant say how you could improve it as I was very pleased with the whole experience.
