For a person my age, it was very difficult to fill out the new patient forms. The print is small and the color of it is a light blue so it took a long time to take care of. There were questions that I didn't think were necessary to include.

Just want to say Dr Angela Rake is absolutely wonderful at what she does and I am so grateful for her and her staff!!~ Christy who worked with her the first time was amazing at making me feel comfortable, as I am a highly anxious person. Just cannot thank them all enough!
