We are expected to be able to work every unit, following the rules and schedules that are in place, but when the MHTs are floating every single shift it makes it impossible to properly execute the schedules that the units have in place. We have “assigned units”, but never work them. So the expectations are difficult to meet when you’re trying to memorize a 12hr schedule for 4 units.I’m not sure if it’s an error, or if this is just a concern, but there have been a few issues with providing support to staff while they are in seclusion with a patient. Unless you’re screaming, you can’t hear when a staff member needs help, and sometimes there is not staff around to hear the screaming to come and assist them. I think the use of walkie talkies or something similar would be beneficial.

Providing more dedicated office or desk space would allow staff to feel they have a place of their own, helping them feel more integrated into the team and better able to complete tasks efficiently.Improve the intake process, as delays or inefficiencies at intake impact all downstream workflows and patient care. Streamlining this process would benefit both staff and patients.Have readily available patient education handouts on common topics (sleep hygiene, MAT for AUD/OUD, BPD, depression, anxiety, nutrition, etc.). This would help staff provide consistent education and resources to patients.Create a process to hold management accountable, including an anonymous reporting system so staff feel safe raising concerns without fear of retaliationThere are ongoing errors in the nursing process, especially related to labs (missed labs, wrong labs, or labs not being sent). These issues do not seem to be addressed consistently, and there is a lack of accountability from nursing leadership. Improving the lab process and holding staff accountable would strengthen patient care and safety.Medication safety needs improvement. There are frequent errors such as giving Trazodone and Hydroxyzine together or administering PRN medications for the wrong indication. Additionally, we need clear call orders for blood pressures to ensure consistent and safe patient care. Greater accountability and education for nursing staff would help address these concerns.

There are too many call ins without any consequences. This makes it hard on those of us that actually come to work. Make people accountable. Often things are implemented before it is ready to be implemented.
