Upon inspection of the patient's chart, she enrolled in SweetSpot via digital consent on 02/06/2026. She was relayed to our troubleshooting department due to some connection issues with her Libre which were resolved. Her first review was completed on 03/16/2026 and it looks like she had multiple phone calls with one of our CDSs in the ensuing days of March. I do see that she was unenrolled from SweetSpot on 05/21/2026 stating financial concerns as the reason. She was under the impression that it was a nurse from Dr. Rauba's office. She is stating she has text message proof that the calls were initiated from "them" and not from her. She also stated she is willing to get a lawyer involved.

This narrative is from a coworker regarding her daughter who is a patient of Dr. Baskar.Patient is very upset about an instance in which she states she called and left a message with a nurse earlier this week with concerns and no one called her back. She said she called this morning to see if Dr. Baskar ever addressed her concern and she was told that Dr. Baskar had said to go to the ER, 3 days ago and no one had called her to let her know that. Yesterday her husband was at home from work and could have watched the kids while she went to Express Care and today, she has no one to watch her kids.She is currently pregnant and having a respiratory infection and with her last pregnancy she ended up with pneumonia, so she is extremely upset that no one called her back earlier this week.

Judy (spouse) has called 3 times on the account of Carl Schwarzer ACCT# 33688. Judy was told they received a bill for copay which is set by insurance. She called back and stated this was AWV. I sent a query to coders and they stated patient had an OV as well as AWV. Labs and medications were discussed concerning his chronic illness. Judy called a 3rd time and stated the provider initiated the discussion on the labs and medication for the patient’s chronic illness. Judy stated they are contesting this bill and they are not going to pay it. This complaint was sent to a billing supervisor and she confirmed everything was done correctly for billing.

Tara reached out regarding visit#3074733, she is disputing the charge as she states was only seen for annual wellness visit and not office visit. I checked with coder and per coder Amanda, patient was seen for a chronic condition and labs, i have relayed info to patient, she disagrees.
