Patient had a follow up appointment with Dr. Middlemist 04/22/26 to discuss the surgery on her right shoulder. During this appointment the patient discussed her right shoulder and her left shoulder, and the provider scheduled another surgery for this. Patient contacted the billing department and complained that she should not have received a bill for this follow up visit. Claim number 3316515 was sent to coders for review. They reviewed everything and determined the coding was correct. They reviewed Dr. Middlemist's notes and learned that additional items were discussed other than the surgery on the patient's right shoulder. This information was relayed to Barbara on 06/16/26 and 07/09/26. She is not satisfied and would like for someone to call her concerning this issue. She does not feel she should have to pay the $30 copay.

Patient called questioning office visit charge on claim 3077424. Claim has both office visit and preventative visit codes. Claim was reviewed by coding, coding stated 'The patient was seen for an AWV, however there was an additional problem discussed and new medication started so that justifies the OV charge'. After relaying this information to the patient, he stated that he did not start a new medication and nothing was discussed outside of normal wellness visit.
