The authorization for advertising and promotional use should appear at the beginning of the survey, before patients provide feedback out of good will to help the functioning of the medical system. The advertising and promotional use question should have an option to say "no" if it is going to remain a mandatory question.The initial intent of the survey seems to be for quality improvement. But at the end end of the survey, the purpose seems to be a marketing ploy that feels manipulative and can result in a breach of trust with patients.
