Dr. Nagda corrected my right surgery that was performed by another doctor. This the reason why I returned for my left shoulder now. The “Other” option should be added under question 4 to enable to provide the reason why returning to see Dr. Nagda.

I was traveling and needed to setup up future scans etc for the tiny window that I am back in Wash DC area, before heading out again to a remote part of the world.
