As most facilities short staff is chronic. Definitely a concern here.Better memory care training for the caregivers. They need to be more aware of how certain things affect dementia patients.

I love the care my stepfather is getting here. Everyone who directly cares for my stepfather does a fantastic job. They deserve lots of kudos for the very hard and difficult work they do. I find the food boring, but I don’t think he cares much. I’m sure it’s nutritious. I’m frustrated with the facility’s phone service, no one answers the phone. I’m extremely unhappy with the unexplained, unsupported 100% increase in level of care fees. It feels very exploitative. I'm also unhappy the management let go a very good Director of Nursing for no apparent good reason. Overall, I am so impressed and thankful for the care my stepfather receives directly from the caregivers. I am unimpressed and disappointed in the corporate attitude and management of the facility.

Turn over of staff is terrible. Short staff most of the time.The previous nurse Nicole was good and caring. Not the same anymore. The staff don’t wipe residents when they change them. Staff checking on residents every 4 hours instead of every 2 hrs. Staff caught sleeping on evening shifts.

Lori Carlile has been such a help in my husband transitioning from skilled nursing to memory care. She has gone out of her way to make this hard decision as easy as possible and welcome us as new family. Her professionalism and manner is outstanding and quite an asset to your organization.

My Dad is a level 3 resident. Since January 2025 - when visiting my Dad, almost always he is soiled down to his bedding. There are no clean linens or blanks and what little supply he has is in his laundry bin. About 70% of his blanks, towels and sheets and clothes have gone missing. I have bought several new (>10@) Henley tops and bottoms as he likes only certain kinds- those have gone missing. I have labeled them and sharpied to no avail. I have found him soiled and unclothed at times- I usually visit around 11am. Once we initiated hospice for the medical care piece—as the staff and on call doctor were unable to administer Advil/tylenol for a common cold and fever— his care has almost become non existent. I have been told that once hospice is initiated that SB refuses to give him showers and baths “and only hospice does”. I thought this was hospice protocol but it isn’t- it’s SB’s protocol. Sadly, I suspect that he rarely receives showers at all - I have voiced my concerns and requested showers at least 1x per week but it doesn’t seem to make a difference. I am on my 4th round of buying linens and clothes. At this point we are considering different options.
