Everything was good except the fruit juice was not good. CNA's & Rn's are friendly. I will miss all the people here.

My experience in the facility is very good the care provided from the nurse are very good. I certainly recommend the facility, And if I will need again SNF later I will definitely will come back.

The facility is wonderful place to recover and get rehab. All the staff are very kind and always had a smile on their face. I will come back to visit everyone. I will recommend the rehab.

We want to thank Jazz for really accommodating my mother's meals from day one. We really appreciate everyone for all care provided for my mother. CNAs at night Lebita and Rengina are very helpful. We have Excellent experience stayed in Sunnyvale Post Acute Center

For the hours I stayed with my father I could see that he was very comfortable in his room and the staff that is a big plus.

My fathers stay was tremendous. He was always taken care of and staff always went above and beyond for him. I'm happy he came here before he went home with me. The only improvement would be the food. I brought him food most of the time.

Katrina & Eleanor were amazing! They were there for my grandpa. They were so kind, patient and sweet to him. I appreciate the hard work ethic and dedication. They deserve a raise or at least a "Good Job" pat on the back. I wish they could come to the Forum with my Grandpa.
